How to apply

Please print and fill in:  Application Form

                                     Emergency Contact Form

                                     Terms and Conditions.

You must provide a total of 4 passport photographs with your application, sticking one to the box on the application form in the top right hand corner.

Also, don’t forget to include your cheque for the £200 bond made payable to CHAMELEON TRUST.

Send your completed forms and the cheque to us at our home address:

Make sure you have included:

1. -3 completed forms: Application Form, Emergency Contact Form, Terms and Conditions
   

2. -FOUR passport photos
   

3. -a cheque for £200 to Chameleon Trust
   

- Please ensure you have paid the correct postage amount.

If for any reason it is difficult for you to return your complete application at present (for instance, if you are on travels out of the UK), then please let me know by email the date you can send the information.

If we have our full quota by the time you contact us, we will inform you of the situation and give you the option of joining our reserve list.
